PRIVACY ACT STATEMENT:  The authority to collect the personal information requested in Questions 5 - 10 is derived from 5 U.S.C. 301 Departmental Regulations.  Furnishing the information to the ACS staff is completely voluntary.  You are not required to provide the personal information to submit your issue.  The information will enable us to contact you if we have a question or need clarification of your input.  If you would like to participate in the upcoming Army Family Action Plan Forum, we will need the information in order to register you as an AFAP Volunteer and to contact you about dates, times, training, etc

You may submit your issues via email to Polk.AFAP@conus.army.mil.  Please be sure to include your suggested issue, detailed explanation of your issue and a description of the changes you recommend.  Also, for any possible clarification that may arise pertaining your issue, please be sure to include your name, telephone number or email address.  Your identity will not be attached in any way to the issue that you submit.  It will be used for issue clarification purposes ONLY.  If you have any questions, please contact the AFAP Program Manager at 337-531-9421.

SCOPE: 
WHY IS IT A PROBLEM?
RECOMMENDATION(S): 
SUGGESTIONS ON HOW TO FIX THE PROBLEM – NO MORE THAN 3 RECOMMENDATIONS.
ISSUE SHEETS CAN BE SUBMITTED ON LINE OR PRINTED AND TURNED INTO FAMILY READINESS CENTER, BLDG 924, CORNER OF MISSISSIPPI AND BELL RICHARD.
ISSUES WITHOUT A NAME, UNIT AND TELEPHONE NUMBER WILL NOT BE CONSIDERED.

ISSUE HAS BROAD IMPACT AND IS WITHIN THE PURVIEW OF FORT POLK, FORSCOM, OR HQDA TO INFLUENCE.

ISSUE IS ATTAINABLE AFTER CONSIDERING CURRENT POLITICAL AND RESOURCE ENVIRONMENT.

ISSUE HAS MEASURABLE OBJECTIVES WITH IDENTIFIED END PRODUCT. 

ISSUE DOES NOT DUPLICATE AN ACTIVE HQDA AFAP ISSUE OR AN ISSUE THAT HAS BEEN DETERMINED UNATTAINABLE IN THE PAST THREE YEARS.
